it is a bug. it happened to me once. the screen keeps shaking left to right cycle. like a see-saw. and when i get into the car, black smoke comes out from the car engine and the car runs by itself. just restart the game.

It is not the problem of your hardware parts but it is a shaking camera problem (commonly after Nico drinks).. I would suggest you to download no camera shake patch or buy the original game.

I'm using GTA 4 version 1.0.4.0 (steam) with ICEnhancer being the only mod. I can get the game to run but it is unplayable no matter what visual settings I use through GTA/ICEnhancer. The map appears and disappears at random, as well as everything in the pause menu. Also the screen flickers constantly and I get very low FPS.
